Note: If an academic unit is interested in offering a credit program completely online, please complete this form and consult with the Office of Online Education:

The purpose of this form is to collect requests from Faculties/Schools interested in developing an existing for-credit program for online delivery, to support strategic planning and the instructional design, development, and implementation process.
